Six sections carry a case from identification to close-out, with the three-level severity criteria and every field mapped to a KPI from Chapter 8. I have included the completed example from Chapter 5, the cracked cistern logged on market day, so you can see what a well-documented case actually looks like.

Stakeholder Consultation Checklist
The co-design checklist from Appendix B, in three phases: pre-consultation planning, session execution, and post-consultation documentation. It includes the eight questions I ask communities when designing a mechanism, and the four minimum specifications every consultation plan must meet, starting with separate sessions for women and workers. Use it for the initial design and again at every redesign.

The full policy template from Appendix C: purpose, scope, definition of retaliation, protections, reporting channels, consequences, communication, monitoring, and review. Every point that needs adapting to your organization sits in square brackets, so you can take it to legal review in a day rather than drafting from a blank page. Adopt it before the mechanism accepts its first grievance, not after.

Cost of Conflict Calculation Worksheet
The worksheet behind the Chapter 2 business case: direct, indirect, and opportunity costs, totalled and compared against the annual cost of running the mechanism. The worked example is the three-week blockade at a copper operation that consumed over US$20 million against a US$240,000 mechanism budget, a ratio of 85 to 1. Fill it in with your own most recent dispute and take the ratio to whoever holds the budget.

90-Day Implementation Roadmap
Appendix E as a single landscape planning table: foundation in days 1 to 30, design in days 31 to 60, operation in days 61 to 90. Every action names an owner, the tool that carries it, and the chapter to read when it stalls, because an action owned by "the team" is owned by no one. Day 90 gives you a minimum viable mechanism; the roadmap also tells you what to do on day 91.

Grievance Mechanism Health Check
The twenty-question scorecard from Appendix F. Sit down with your grievance log and last quarter’s data, score each question 0, 1, or 2, and be harsh: the questions are evidence-anchored, so "yes, but I could not show you" scores 1. Three bands read your total and route you to the chapter that repairs the weakest point first, whether that is redesigning with the community or letting the data set the repair order.
How to use them
If the mechanism already exists, start with the health check and your last quarter’s data, and let the three weakest answers set the repair order. If you are building from scratch, start with the cost worksheet, because it wins the budget conversation, then follow the roadmap: the registration form, the consultation checklist and the non-retaliation policy each have their day on it.
They are free to use in your own organization and with your own clients. Please keep the attribution. They are working templates, not legal advice: adapt the bracketed provisions with qualified counsel before adoption.
